HonorVet Technologies is seeking a travel nurse RN Care Manager for a travel nursing job in Apple Valley, California.

Contract Length: 16 weeks
Shift: Days, 5x8 (8:00 AM – 4:30 PM)

Job Requirements

• Minimum 2 years acute care case management and discharge planning (required)
• Prior travel nursing experience required (no first-time travelers)
• Active RN license
• Certifications: BLS (required)
• Charting: Epic (required)
• Patient ratios: Days 1:25; Weekends 1:45
• Must have hospital experience (behavioral health experience alone does not qualify)

Facility & Unit Details

• Unit: Case Management (Acute Care Hospital)
• Caseload: Up to 25 patients daily
• Orientation: 40 hours
• Weekend rotation required (minimum 4 weekend shifts per 6-week schedule)
• Patient Age Groups: Neonates, Adolescents, Adults, Geriatrics
• Interdisciplinary Support: Physical Therapy, Social Services

Skills Needed

• Assessments, care coordination, discharge planning, utilization review
• Organize and participate in family meetings
• Arrange DME, SNF, and ambulance transport independently
• Experience with HMOs, IPAs, adoption/foster care, substance abuse treatment, homelessness, advance directives, POLST, and protective services
• Knowledge of CMS, Joint Commission, DRGs, and regulatory standards
